On Feb. 15, 2023, the American Academy of Sleep Medicine released the AASM Manual for the Scoring of Sleep and Associated Events, Version 3. Based on the recommendations of the AASM Scoring Manual Committee, the AASM Board of Directors approved several changes in this update to the AASM Scoring Manual. All AASM-accredited sleep facilities are required to implement the new rules in Version 3 by Dec. 31, 2023.

The AASM Manual for the Scoring of Sleep and Associated Events is the definitive reference for the evaluation of polysomnography (PSG) and home sleep apnea tests (HSATs). This comprehensive resource provides rules for scoring sleep stages, arousals, respiratory events during sleep, movements during sleep, and cardiac events. The AASM Scoring Manual 3 also provides guidance on standard montages, electrode placements, and technical and digital specifications.

Changes in AASM Scoring Manual Version 3

Both new and updated rules and notes are included in AASM Scoring Manual, Version 3. The most notable changes include changing the scoring of hypopneas using ≥4% oxygen desaturation from pre-event baseline from ACCEPTABLE to OPTIONAL, adding new RECOMMENDED rules for scoring the presence of 2nd or 3rd degree atrioventricular heart block and cardiac pacemaker rhythm, and adding a new section with reporting rules for the multiple sleep latency test and maintenance of wakefulness test.
