On Sept. 21, 2026, the Centers for Medicare & Medicaid Services (CMS) announced that 2025 participants in the Merit-based Incentive Payment System (MIPS) can now view their performance feedback, including final scores and payment adjustment factors.

Your final score determines the payment adjustment you will receive in 2027. A positive, negative, or neutral payment adjustment will be applied to the Medicare-paid amount for covered professional services furnished in 2027. The 2027 MIPS payment adjustment information is expected to be available next month.

Follow these steps to access feedback

  1. Sign in to the Quality Payment Program (QPP) website using your HCQIS Access Roles and Profile (HARP) credentials, the same credentials used to submit your 2025 MIPS data.
  2. Click “View Feedback” on the home page, then choose your organization. Practice representatives can view individual, subgroup, and group feedback. Third-party representatives cannot access final feedback or payment adjustment information.

If an entity believes that there may be a discrepancy in the calculation of its final score, it may request that CMS review the calculation through a process known as targeted review. The targeted review period will remain open for 60 days following the release of final scores.

Examples of previous targeted review circumstances include:

  • Data submitted under wrong Taxpayer Identification Number (TIN) or National Provider Identifier (NPI).
  • Eligibility or special-status issues. For example, you have Qualifying Alternative Payment Model (APM) Participant (QP) status and should not receive a payment adjustment.
  • Performance categories were not automatically reweighted, even though you qualify for reweighting because of extreme and uncontrollable circumstances.

This list is not all-inclusive, and there may be other reasons why a targeted review is necessary.

Follow the steps to request a targeted review

  1. Sign in using your HARP system credentials. These are the same credentials used to submit your 2025 MIPS data.
  2. Click “Targeted Review” on the left-hand navigation menu.
