The Centers for Medicare & Medicaid Services (CMS) has released a new version of the form required for submission of paper claims. CMS will begin to accept the new CMS-1500 form (version 02/12) on January 1, 2014. The revised form allows for providers to indicate whether they are using ICD-9 or ICD-10 diagnostic codes. It also includes space for the provider to report more diagnostic codes on each claim – an expansion from four to twelve possible codes. CMS requires that most providers submit claims electronically. However, some providers are exempt from this requirement and are still given the option to submit claims using the CMS-1500 paper form. Providers should visit the CMS website to determine requirements for the electronic claims exemption.
