Case
A53-year-oldmale is referred to the sleep clinic for suspectedobstructive sleep apnea
hypopnea syndrome (OSAHS) and excessivedaytime sleepiness (Epworth scoreof 15/24).
Hehas loud snoring,witnessed apneabyhiswife, awakens himself by snoring/snorting and
wakes routinelywith aheadache anddrymouth in themorning.He also sometimes awakens
gasping for air. Hedozes off accidentally at his deskduringwork andhas dozedoff for a
momentwhiledriving, despite a consistent and adequatedurationof sleep. Hehasmild to
moderate restless leg sensations in the eveninghours. Hehasnoparasomnias or ancillary
symptoms to suggest narcolepsy.
Physical exam:
BP115/65, Pulse 94, RR18,Ht 163.8 cm (5’ 4.5”), Wt 71.396kg (157 lb6.4
oz), SpO297%, BMI 26.60kg/(m^2)
HEENT:
Large tonguewith scalloping, high archedhardpalate,MallampatiClass IV, no
tonsils,mildoverjet, narrowbut patent nasal airway and17”neck.Neuro, cardiac and
pulmonary exams werewithinnormal limits.
PMH:
Hypersomnia, snoring, restless leg syndrome, hyperlipidemia, hypertension,GERD
