1. Preparation: Be sure your intakequestionnaire includes
questions that evaluatepossible insomnia and someof the
comorbidities; severityquestionnairesmay alsobehelpful
in followingupprogress. Some examples include:The
InsomniaSeverity Index (Bastien et al. 2001), PROMIS
™SleepDisturbance Index, orPROMIS™SleepRelated
Impairment(Yu et al. 2012;Arnedt 2012).
2. Build referral systems and test them; thismay include
psychiatrists, gynecologists (treatinghormonal imbalance
sometimes improves insomnia), primary carephysicians and
others.
3. Consider dedicatingone clinicday to treat only insomnia
patients.Newpatient visitsmaybe longer indurationon this
day to ensure adequate assessment.
4. Keep information aboutwhomight be certified inbehavioral
sleepmedicine inyour areaonhand. Seehttp://www.
behavioralsleep.org/FindSpecialist.aspx for an example.
5. Resources: In the absenceof a clinician towhom thepatient
canbe referred, there arewebsites tohelpwithdifferent
psychiatricproblems (mostly anxiety andmood), aswell
aswebsites tohelpwith sleephygiene andbasic insomnia
treatment recommendations.
Online InsomniaTreatments
For practitionerswithout access to skilled clinicianswho are
certified inBehavioral SleepMedicine, onlinedeliveryofCBT-I
maybe an effective alternative approach.Thenumber of studies
supporting the valueof online interventions for insomnia,
particularlyover the last year, has increased significantly. For
